Taos Forms
Please chose the Taos form with the appropriate body part as stated by the office staff.
Directions for filling out Taos Forms:
* Taos forms are functional indexes to let us know how you are currently doing. They give us a starting point to compare to at the end of your treatment sessions.
* Under each category, check the box that most accurately describes what your functional level is. If the item does not pertain to you, you can leave it blank.
* At the bottom of the form, put how many days/months/years the injury occurred.
* On the pain scale, place a "X" where your pain level has been within the past 24 hours.
* Make sure to bring this form with you to your visit.
Cervical/Thoracic Taos Form (neck & upper back)
Lower Extremity Taos Form (ankle, knee, hip)
Upper Extremity Taos Form (elbow, shoulder, chest)
